التأمين الصحي يحدد الأوراق المطلوبة وخطوات استخراج البطاقة العلاجية للموظفين

حددت الهيئة العامة للتأمين الصحي الأوراق المطلوبة وإجراءات استخراج البطاقة العلاجية للعاملين بالقطاعين الحكومي والخاص.
يتطلب استخراج البطاقة إجراء الكشف الطبي الابتدائي، واستخراج استمارة الاشتراك المميكنة، ثم التوجه لمركز إصدار البطاقات.
تشمل المستندات المطلوبة نموذج الكشف الطبي، واستمارة الاشتراك، وصورة بطاقة الرقم القومي، وصوراً شخصية، وبرنتات تأمينية للموظف والمنشأة.

كشفت الهيئة العامة للتأمين الصحي عن إجراءات استخراج بطاقة التأمين الصحي للعاملين بالقطاعين الحكومي والخاص ، والمستندات المطلوبة للحصول على البطاقة العلاجية والاستفادة من الخدمات الطبية التي تقدمها الهيئة للمؤمن عليهم. وأكدت هيئة التأمين الصحى أن استخراج البطاقة يتم من خلال التوجه إلى مركز إصدار البطاقات
